Its likely the tiny oring that seals the piston stem. If memory serves it was 007 urethane. Its a pain to get to stay in when you push the piston in. I usually back it with an Allen key while you push the piston through the top. Holding the oring until you get the piston through. Also that piston looks really badly gouged at the base you may get reg creep. -
O ring #6 or #11 are the only two that will allow air to vent through that hole. A lot of people don't even know that #6 is in there.
The only other thing I can think of is something is wrong with the shim stack to the point that the whole piston is not sitting in the correct spot

Reg stack looks okay but I will redo it. The reg seems to hold air for a moment and the pops.Comment

Check the output pressure. If the seat is bad and it's just allowing tank output pressure through the spring stack may be compressing enough to allow the larger piston o ring to clear the vent hole

Those two orings already mentioned are the only ones preventing air from venting through that hole. If those are sealing, then something else may be going wrong (the too-short/spring stack is one that I haven't heard of but seems possible). If you still have issues, it may be helpful to pull it apart and take a picture of all the components separated.Comment

Ok and? Just because it's new doesn't mean it's working properly, it's still worth putting a reg tester on there too verify and that will give you more info than you have nowTrbo323 - put in a brand new reg seal.
I was missing one bevel spring on one of my regs at one point, took me forever to sort that out. Double check the count and orientationThose two orings already mentioned are the only ones preventing air from venting through that hole. Trbo323 I am attempting to put it on a reg tester but it won’t hold air and vents, so no pressure reading. Want to preset the HPR to avoid any issues.Comment

Are you starting with the adjuster all the way in?

Trbo323 I’ve tried it flush at the bottom and tried it about half way out. I can try it all the way out.Comment

All the way in is 0 pressure. I would start there

Ok, understand that everything I'm about to suggest is pretty low probability so sorry if they feel like dumb questions but I feel like you have gotten to the point where it's either something stupid that got overlooked or something complicated enough that nobody else has come across it yet
1) I'm still pretty suspicious of that 007 o ring that is inside the regulator body shaft. Now that I think about it, it's part number 6 but it's a size 007. Any chance you used a 006 instead?
It also has a grove that it needs to be seated into, it somewhat "clicks" into place. From what you are describing I'm wondering if it's not seated all the way in the groove. So when you pressurize it holds for a moment and then that ring moves out of position allowing air into the bevel spring stack.
I would carefully double check that ring both for size and where it's positioned. If you take the springs off and push the piston into it's position does that ring get pushed out the bottom of the shaft? It shouldn't. I've also seen one case where an old 007 o ring had disintegrated but still had bits left in the grove which played havoc when a new ring was tried to put into the same spot.
2) it shouldn't matter but what input pressure are you using?
3) is there any chance that's not the original piston? Idk if dye changed the piston length or anything between the 2 and 3 but, just a thought. Have you had this reg working in the past
Something else to try, get a second reg set up to feed into the hyper, set the pressure to whatever you want the output of the hyper to be and then feed into it. This will rule out an overpressure situation if it still vents. If it seals then it *might* mean something is off where it's acting like it's overpressure even though it's not
