I've seen this several times before when I use to resell paintball gear, primarily for valves like Autocockers where you have to push it past some threads. I suspect that it gets a nick on the side, drys out, and then the compression causes it to split at the weak points.

I figured that was what had happened since it did have to come out over some treads and with no lube. I also thing the o-rings were oversized as well.

This looks like a spiral failure, it happens when a ring is installed improperly (rolled) in addition to/or if a ring is pushed through the tube slowly (and usually without enough lube) where a section of the ring catches on the metal and rolls enough that the rubber shreds itself. To prevent that make sure the tube the valve is being pushed through has lube on it (oil usually works but some lightweight silicone grease can also help) and that the o-ring is also lubed. If you have to pass the valve over threads and it is a snug/tight fit you can pack the threads full of grease to allow the ring to move over them smoothly and then clean the threads out afterward.

I've seen similar O-ring failures with old rubber O-rings and heat... almost as if the O-ring started to melt enough to bond to the indie of the tube.

I agree that one is probably from being installed twisted, and then jammed past some threads with reckless abandon.

Filling the threads beforehand is a smart idea when you have to move the valve through them.
