I made one.
I'll have to go look it up again to be sure but if I remember right
3/8 socket drive adapter and drill a hole right in the middle for the valve stem. I forget what size that hole is though. Start small, work your way up.
I also had to heat up my drive adaptor to get it to lose some hardness first

That style changer is called a quicksilver irrc and Glenn was selling them through PPS.

I'm sure a brass or mild steel version would do most of the time, but I recently discovered my old one is pretty tough. I thought I would put it in my little lathe and put some knurling on it. My knurling tool didn't scratch it. That explained how I bent the T-handle many years ago without any other failures.
